Loose comments like "it's fine" or "doesn't work" are hard to act on. A short, fixed form gives you answers you can compare across testers. Copy the fields below into any form tool, a shared document or an email.

Why these fields
- What they tried tells you which flow was covered. Gaps show what nobody tested.
- Works / problems / did not work is easy to count.
- Expected vs actual separates bugs from unclear design.
- Steps let you reproduce the problem.
- Device and version explain "only on my phone" reports.
- Severity helps you sort, see how to prioritize app feedback.
Example (invented, for illustration only)
- Tried: creating a new list. 2. Works with problems. 3. Save button did nothing the first time; I expected a confirmation. 4. Open app, tap +, type name, tap Save. 5. Example phone, Android 14. 6. 1.0.3. 8. Annoying. 9. Show a confirmation.
Adapting it
- Cut questions that do not apply. Fewer than ten fields keeps answers complete.
- Add one app-specific question per main flow ("Did the onboarding make sense?").
- Do not collect personal data you do not need. Tell testers what you do with their answers. This is not legal advice; check privacy rules that apply to you.
- Add a short note on how to find the app version and take a screenshot.
